REMINDER- School-Linked Health Services Information
Important Information for TSD Families:
Talawanda School District is pleased to continue to offer school-linked health services with both Primary Health Solutions and Oxford Pediatrics|TriHealth this school year. We recognize that with winter approaching, we often see an increase in need for medical care during the cold and flu season.
This partnership provides an opportunity for Talawanda students to go to either provider during the school day for routine well-child checkups, medication checks, or for any other acute illness (ear infection, sore throat). With parent/guardian consent, adults are welcome to attend appointments with the child but are not required to be there. Talawanda School District can assist in connecting students to these health resources through a Rural Communities Opioid Response Program Implementation Grant from the Health Resources and Services Administration that can provide transportation when needed with the appropriate parent/guardian signed waivers.
If you choose to enroll your student in school-linked health services, a consent packet and medical history must be completed by the parent/guardian. Please choose from either of our community partners. Hard copies of the forms can be provided in the main office. Completed forms can be returned to the main office/school nurse.
